Assessment
To ensure our services align closely with individual needs, we begin with a comprehensive pre-admission assessment.
During this process, we thoroughly review an individual’s care history to identify the most suitable support. We evaluate their current needs, including any potential challenges related to behavior. Additionally, we assess compatibility with existing residents and the individual’s ability to integrate successfully into the local community. Our goal is to align routines, relationships, and hobbies with an appropriate service or home, facilitating a smooth transition.
We actively consult with the individual, their family members, social workers, clinicians, therapists, and others involved in their care. This collaboration is essential for gaining a clear understanding of their accommodation and support needs. Through these consultations, we may identify specific adaptations required to optimise our services for the individual.
Our person-centered approach empowers individuals to take greater control of their lives and actively participate in decisions that affect them. In partnership with housing providers, we offer a variety of Supported Living environments tailored to meet the diverse needs and preferences of those we care for. This includes the possibility of transitioning to more independent accommodations as their needs evolve and independence grows.
The pre-admission assessment allows us to understand each individual’s unique requirements and determine the appropriate level of support. By working collaboratively, we aim to provide opportunities and choices that enhance their care experience and empower them to reach their full potential.
Admission
The transition to our services is tailored to meet each individual’s unique needs. For some, this process may be straightforward, involving a simple visit before moving in. For others, it may require a series of meetings with the CCRT support team, gradually introducing new faces and familiarizing them with the existing service before the individual relocates to their new environment. Regardless of the situation, we ensure that the transition plan is centered around the individual to facilitate a smooth and seamless experience.
Our commitment is to provide consistent, high-quality support that is both appropriate and responsive. We employ a ‘key worker’ system, where each individual is assigned a primary contact who quickly becomes familiar with their care and support needs. Dedicated support teams are established to ensure that individuals are well-acquainted with the staff who provide their support. Additionally, we implement contingency plans to address staff absences or unforeseen disruptions in service delivery.
From the moment of the initial assessment, we create and develop a personalized care plan to ensure that specific needs are met upon moving into the new home. These care plans are living documents, continually evolving to reflect the individual’s progress and changing requirements.
